We are looking for an experienced qualified social worker to join our clients busy multi-disciplinary Intermediate Care Team. This is a temporary contract outlined to run until March 2010, however it is likely to run over this initial period, whilst the team recruit for permanent members of staff and internal promotions are completed.

The Intermediate Care Team is a partnership of health and social care professionals, working together to prevent the unnecessary admission or re-admission of older people into hospital, and maximise independence in the community.

Working as part of a team, the successful applicant will undertake assessment and care management as part of a rapid response to individuals who are in urgent need of support after a hospital discharge. You will join a friendly and supportive team, and will receive regular supervision and appraisal, together with encouragement and opportunity for continuous professional development.
