Band 8 Pharmacist required for a 3 month period to work a minimum of 20 hours a week, you must have experience of working in a PCT providing advice on handling and the use of pharmaceuticals, CD regulations and guidance, writing and reviewing medicines policies, SOPs and Patient Group Directions. Positions start on the 2nd Nov 09.

Documentation required for registration

• Proof of Identity ( either Passport, Drivers Licence or Birth Certificate)
• Proof of eligibility to live and work in the UK (if not UK or EU passport holder)
• 2 x Proof of address (dated within the last 3 months)
• Up to date CV (including full employment history, since leaving full time
education)
• Qualifications (copies of all relevant certificates)
• Evidence of registration with professional body (HPC/RPSGB number)
• 3 passport photos (signed and dated on the back)
• NI number (original NI card, P45 or P60
• Mandatory Training Certificates (dated within the last 12 months)
• Vaccinations (proof of immunity for Hep B, MMR, Tuberculosis and Varicella)
